A questionnaire asking students about things ranging from drug and alcohol use to sexual histories has gotten the teacher who administered it placed on leave. A teacher at Roy High School gave students a test that results would indicate the student's level of purity or indecency.

This sent red flags up for one student's mother who was wondering just what exactly her daughter was learning in the "Adult Roles and Financial Literacy" class. The student's mother, Heather Danks Miller, said "I was in shock. I couldn’t believe something like this was handed out to students." The survey says the questions from a 1981 Dear Abby column and is 30 questions long. Some questions ask, like question 27, "Have you (your girl) ever had an abortion?" Another question, question 28, asks "Have you (your girl) had more than one abortion?"
The questionnaire had some other questionable content, such as the questions which gave more "indecency" points to experimenting with a member of the same gender than it did to slipping drugs into someone's drink. Some other phrases in the test include, "Even though you are straight would you go kinky to see what it's [sic] like" and being "kissed against your will." The school officials made a statement saying, "Although we strive for a standard of perfection in teaching, occasionally mistakes are made. In such cases, it will be dealt with appropriately, always with the best interests of our students in mind."
